New/ Change of P&C Directory Info Form

This form can be used to inform the P&C of changes to the school directory. Fill in your child's name and class and then only the new information to be updated:
  • Child's Info

  • Parents' Info

  • Change of Information Details

  • Confirm which info is to be changed. eg: delete address details or eg: change Mum's mobile to number indicated above.
